It’s important to plan for your family’s financial security in case the unexpected happens. That’s why Rollins provides basic life insurance and accidental death and dismemberment (AD&D) insurance for employees — at no cost to you.

During Open Enrollment, you also have the option of buying Voluntary Term Life (VTL) and AD&D insurance for yourself and your family.

Basic Life and AD&D Insurance

Rollins provides basic life and AD&D insurance through Voya to assist you and your family in the event of a death or dismemberment. These benefits are fully paid for by Rollins.

You are automatically enrolled in basic life and AD&D insurance. However, you will need to choose a beneficiary when you enroll in benefits to ensure your wishes are carried out if something happens to you.

* Under current tax laws, you are required to pay income taxes on the "value" of your company provided basic life insurance coverage in excess of $50,000. The "value" is determined by your age and schedule established by the IRS. This tax liability is called "imputed income."

Voluntary Term Life (VTL) and AD&D Insurance

You can choose to purchase additional life insurance for yourself, your spouse, and your children. You pay the full cost of any supplemental life insurance and/or supplemental AD&D insurance coverage.

If you do not sign up for Voluntarily Term Life Insurance when you are first eligible, or if you choose to increase your current coverage amount, you will be subject to Evidence of Insurability (EOI).

Voya will review the Medical Questionnaire and determine if you will be approved for Voluntary Term Life coverage. Voya can deny coverage at this time.

Coverage can be denied if you have a current or past medical condition, or are taking medication for a medical condition.

All elections can be made independently of one another. For example, you don’t have to elect VTL in order to elect coverage for your spouse and/or dependents.

Your VTL and AD&D Options

Spouse Voluntary Term Life** Your spouse You can purchase coverage for your spouse in $25,000 increments, up to $250,000 maximum. If you and your spouse both work for Rollins, you may be covered as an employee or a spouse, but not both. You have a one-time opportunity when you are first eligible to elect coverage with no questions asked. If you are electing coverage greater than $25K when first eligible, you will be subject to EOI.
Child Voluntary Term Life (to age 26) Your child(ren) You can purchase coverage for your child(ren) in $5,000 increments, up to $20,000 maximum for each child. If both you and your spouse work for Rollins, only one of you may cover your children. No EOI is required.
** At age 65, coverage reduces to 65% of original amount; at age 70 reduces to 50%.

SELECT A BENEFICIARY

It is important to choose a beneficiary or beneficiaries to receive the policy’s benefit payment in the event of the insured person’s death. You should designate your beneficiary(ies) when you enroll for benefits. 

EVIDENCE OF INSURABILITY (EOI)

Evidence of Insurability is a series of health-related questions you answer with the help of your doctor. Depending on the amount of coverage you choose, you may also need to take a paramedical exam.

FILING A DEATH CLAIM of an EMPLOYEE OR DEPENDENT

Contact Rollins Benefits at (404) 888-2325 in the event an employee or covered dependent passes away. Rollins Benefits will begin the claim process with Voya. Voya will then contact the named beneficiary(ies), typically within 48 hours of the claim being filed.
